Long-term Care Discharge Service

We provide information, advice and support to people in hospital who are being discharged into a care home:

• Choice of residential or nursing home care in East Cheshire
• Finance and funding issues, for example capital limits and allowances
• Relevant local and national legislation such as assessments, Power of Attorney and other general issues of concern
• People's rights in relation to long-term care
• All aspects of considering and choosing a home, in a range of free factsheets

The service operates from Tuesday to Friday, 9am to 4.30pm.  Contact 01625 661504.
